What to bring to your first appointment
To prepare for your first appointment, please bring:
- Family doctor name and phone number
- Previous medical history
- A list of medications and allergies
- Your dental insurance card and booklet
Financial policy
Payment is due at the time of treatment. If you need payment arrangements please check with our front desk staff prior to your appointment.
Insurance
We will build your treatment plan and help you submit your insurance claims. Please note that your insurance is a contract between you and your insurer. You are responsible for any amounts your insurer does not cover.
